Docetaxel is a semi synthetic derivative of paclitaxel, which is extracted from the European yew tree as a monomer 10 deacetylated berry gibberellin and is a hydrophilic anti-tumor drug. Similar to the mechanism of action of paclitaxel, it binds to microtubule proteins, promotes microtubule stability, and leads to G2/M cell cycle arrest.
Docetaxel is a paclitaxel class drug that promotes the assembly of microtubule dimers into microtubules, while stabilizing microtubules by preventing depolymerization, blocking cells in the G2 and M phases, thereby inhibiting cancer cell mitosis and proliferation. Docetaxel has stronger pharmacological effects than paclitaxel, with a concentration three times higher in cells and a longer retention time. Its affinity for microtubules is twice that of paclitaxel; As a microtubule stabilizer and assembly promoter, its activity is twice that of paclitaxel; As a microtubule depolymerization inhibitor, its activity is twice that of paclitaxel. In vitro anti-tumor activity tests have confirmed that the anti-tumor activity of docetaxel is 1.3-12 times that of paclitaxel. Clinical studies have shown that docetaxel is more effective than paclitaxel in anthracycline resistant breast cancer. Docetaxel is by far the most effective drug in the second-line treatment of anthracycline resistant breast cancer; Docetaxel is one of the most effective drugs in monotherapy and combination chemotherapy for non-small cell lung cancer. Genetic toxicity: Docetaxel showed a mutagenic effect in CHO-K1 cell chromosome aberration assay and mouse bone marrow micronucleus assay, but no mutagenic effect was observed in Ames assay and CHO/HGPRT gene mutation assay. Reproductive toxicity: Intravenous injection of docetaxel 0.3mg/kg in rats (approximately 1/50 of the clinically recommended dose based on body surface area) did not cause any damage to fertility, but could lead to a reduction in testicular weight. This result is correlated with the results of repeated administration trials in rats and dogs for 10 cycles (once every 21 days for 6 consecutive months); When rats and dogs were intravenously injected at doses of 5mg/kg and 0.375mg/kg, respectively (equivalent to approximately 1/3 and 1/15 of the clinically recommended dose, respectively, based on body surface area), testicular atrophy and degeneration were observed. Increasing the frequency of administration at low doses also showed similar effects in rats. The use of docetaxel during pregnancy can cause fetal damage. During organ formation, rats and rabbits were given docetaxel at doses of ≥ 0.3mg/kg/day and 0.03mg/kg/day, respectively (equivalent to 1/50 and 1/300 of the recommended clinical daily dose, respectively, based on body surface area). Embryotoxicity and fetal toxicity were observed, manifested as intrauterine death, increased fetal absorption, decreased fetal weight, and delayed ossification. The above doses can also cause maternal toxicity. There is currently insufficient and strictly controlled clinical research data on pregnant women. If patients use this product during pregnancy or become pregnant while using this product, they should be informed of the potential harm to the fetus and the potential risk of miscarriage. Women who have the potential to conceive should avoid pregnancy during the use of this product for treatment. It is not yet clear whether docetaxel is excreted from human milk. Given that many drugs can be excreted from human milk and docetaxel may cause serious adverse reactions in nursing infants, mothers should stop breastfeeding before using this product.
